WHO IS ELIGIBLE FOR HOSPICE CARE?
- Those who have terminal illness with a prognosis of six months or less if the disease follows its normal course.
- Those who have primary physician.
- Those who are willing to shift goals of treatment from curative to palliative.
- Those who live within our geographic area.
- Those who have someone available to accept responsibility for 24-hour care at home.
- Those who are experiencing grief or bereavement.
