Anyone can initiate a referral to Hospice of Oklahoma County. Hospice will call
the family to get information and informed consent. Hospice will also call
the doctor for information about diagnosis and prognosis, orders for
medications and treatments, patient and family understanding of the
disease and prognosis, patient history and physical information and orders
for hospice care. A hospice staff member will make an appointment for a family/patient visit and will
develop a plan of care based on the nursing assessment.
